Ground collisions at airports are among the most preventable aviation incidents. Runway incursion detection systems, ground radar, and tower-managed movement areas exist precisely to prevent vehicles from being in the path of arriving aircraft. The systems work — the rate of runway incursion accidents has declined over decades. But decline is not elimination.
Ground vehicle management at airports is a coordination problem, not a technology problem. The runway is clear or it is not. The fire truck was where the jet needed to be. No algorithm, no sensor — a vehicle was present on an active runway during landing operations. The failure is in the procedural layer: who authorized the vehicle, who confirmed runway status, what communication chain broke between the tower, the ground controller, and the vehicle operator.
Aviation safety is built on the Swiss cheese model — multiple layers of defense, each with holes, but aligned so that no single hole allows an accident. Ground collisions happen when the holes align across all layers simultaneously: tower communication, ground radar, pilot visibility, vehicle operator awareness. The accident requires multiple simultaneous failures. Each individual failure is unexceptional. Their alignment is the catastrophe.
The two pilots died not because one thing went wrong but because several unremarkable things went wrong at the same time.
