Hospital wastewater sampled across 64 healthcare facilities in all 16 Polish voivodeships reveals a distributed reservoir of carbapenem-resistant bacteria. Pseudomonas aeruginosa appeared most frequently across all regions, but Acinetobacter baumannii showed greater genetic diversity, with both closely related and unique strain profiles. The wastewater contained numerous mobile genetic elements involved in resistance gene transfer, and bacterial counts correlated significantly with antibiotic concentrations in the effluent.
The finding turns the intuitive picture inside out. We think of hospitals as places where resistant organisms infect patients. The wastewater data suggests hospitals are also broadcasting resistance genes into the environment through their drains. The effluent is not waste; it is a selective medium -- antibiotic residues in the water maintain the exact selective pressures that favor resistant organisms, ensuring that resistance genes survive and transfer long after leaving the clinical setting. The hospital's therapeutic environment and its environmental footprint are running the same evolutionary experiment.
The broader principle is that any institution whose internal operations exert selective pressure will export that selection through its waste streams. The boundary of the institution is not the boundary of its evolutionary influence. What flows out carries the signature of what happened inside, and if the outflow sustains the same conditions that generated the selection, the institution's evolutionary logic extends indefinitely beyond its walls.
